I, being the parent or legal guardian in having legal custody of the above listed child/children, do hereby consent to said minor(s) participation in the VBC activities of Zion Christian Church. I understand that, in the event of an accident involving the above child and medical treatment is required, every effort will be made to contact me. Realizing that is not always possible for the immediate contact, I give permission to the staff/ volunteers to secure the surfaces of a license physicians to provide the care necessary, including emergency medical treatment, anesthesia, and / or surgery for my child’s wellbeing, knowing full well that I will be contacted at the first possible moment for the person deemed responsible by ZCC. I understand that this release is being relied upon by the ZCC and the staff / volunteers of the children; and without this instrument being executed by me the above child will not be permitted to participate in activities related thereto. I do hereby release, discharge, and exonerate ZCC and all persons acting as staff/ volunteers at VBS activities from any and all liability whatsoever resulting from personal injury to the above minor, or damage to the property of the above minor, which may occur, except in the case of gross negligence. I do hereby certify that I assume full responsivity of any act committed by this VBS related to activities resulting in injury or damage to the property of others.

Dear Zion Kids Parents,

We love the incredible experiences your children are having in Zion Kids, and we would be honored to share a glimpse of that joy with our church family and community.

From time to time, we may take photos or short videos during classes and programs to highlight what God is doing in our children’s ministry. These moments may be shared on our social media pages, website, or church app. Please know that children’s names will never be included.

Your child’s privacy and safety are extremely important to us. We will only capture and share these moments with your permission, and we will always respect your decision.
